By providing you with this document, Request Personnel Services Inc. is pledging its continued commitment to protecting the personal information we collect. We want you to understand the purposes for which we collect personal information. We also want to make sure that we have your consent to continue to collect, use and disclose personal information for these purposes.

As an existing candidate in Request Personnel Services Inc.’s databases, we require you to review our Privacy Protection Pledge. If we do not hear from you regarding the information contained in this Pledge, we are implying your consent to the collection, use and disclosure of your personal information as specified in our policy.

Please note that personal information includes any information and identifies you, including but not limited you, your contact details, resume, information obtained through reference and background checks, interview notes about you, educational records, reference letters, and skill testing results.

We use your personal information for the following purposes:

Note that Request Personnel Services Inc. collects information about you from appropriate references regarding employment dates, job responsibilities, performance, attendance, salary history, and other job-related information. This information will be shared with select clients of Request Personnel Services Inc., in order to assist them in determining whether you are a suitable candidate for a specific job position.

In the event that you wish to withdraw your consent to any uses or disclosures of your personal information, it is your responsibility to inform Request Personnel Services Inc. in writing at which time you will be informed by Request Personnel Services Inc., as to whether and how such withdrawal will affect the services being provided to you.
