RESPONSIBILITIES

On the first day of your assignment arrive at least 10 minutes early so you can get familiar with your surroundings and ask the following questions.

Should I be aware of any hazards?

Where are the emergency exits?

What are my break times?

Where are the washrooms located?

1. It is your responsibility to report any incidents or accidents no matter how small to our office and your supervisor. Law requires reporting it.
|
|
|
|
2. Full time placement with a company cannot be done until the employee has successfully completed 480 working hours. At this time Requests client may ask that the employee be brought on full time. You are not allowed to ask our clients to be hired on full time.

|
|
3. Leaving work for an appointment must done so with permission from Request Personnel Services Inc. as well as your supervisor. As a courtesy, you must inform our offices and your supervisor at least 24 hours prior to your appointment.
|
|
|
|
4. If unable to attend work for your next scheduled shift you must notify Request Personnel Services Inc. at least 4 hours prior to the start of your shift. If you are calling before or after business hours, you must leave a detailed voice message including, the date, time, reason for absence, your assignment location, supervisors name and your date of return.

|
|
5. Availability and completion of assignment. Upon completion of the first day of an assignment, please call our offices to inform us how your day went and if you would like to return the next day. It is your duty to inform our offices when your assignment has finished and when you are ready to start another assignment.

|
6. Daily Safety of the Workplace checklist. If required, it is your responsibility to complete the daily workplace checklist at the beginning of your shift.
